Telehealth Maternity Care Program
To support you during this unique time in your life, Sarasota Memorial Hospital is offering a free maternal telehealth program, in partnership with the Florida Department of Health, to provide additional services during your pregnancy and up to one year postpartum.
Who Can Enroll
To be considered for this program, you must live in Charlotte, DeSoto, Manatee, or Sarasota County and be pregnant or in the postpartum period (up to one year after delivery). This program is designed to support women who may face additional medical challenges, including those who:
- Are of childbearing age who are pregnant or recently gave birth and meet one of the following criteria:
- Do not have health insurance and do not qualify for Medicaid
- Are in their third trimester and have not yet received prenatal care
- Have one or more medical or health-related concerns, such as a complex medical history, substance use concerns, high blood pressure (hypertension), or diabetes
About the Program
Once you are enrolled in the program, SMH Nurse Navigators on the Pregnancy Care Team will help connect you to local care and resources, schedule telehealth visits, and provide education and incentives to guide you throughout your pregnancy and postpartum journey. During each telehealth visit, our team will monitor your progress and answer any of your questions. Additionally, our Nurse Navigators have the ability to provide you with a glucose meter or blood pressure cuff, if deemed necessary according to diagnosed medical conditions.

- Virtual Clinical Support – Regular check-ins with maternal health professionals for prenatal and postpartum care.
- Education & Care Guidance – Easy-to-understand materials and classes to help you navigate pregnancy, postpartum recovery, and appointments.
- Appointment Support – Assistance attending and preparing for prenatal and postpartum doctor visits.
- At-Home Health Monitoring – Support and medical devices to track important health indicators from home.
- Social & Community Referrals – Personalized help connecting to food assistance, housing support, and other community and social services.
