COMING SOON: A New and Improved Patient Portal
Sarasota Memorial is excited to announce that it is transitioning to a new patient portal called MyChart, making it easier for you to access your health information and stay connected to your care.
Beginning Oct. 3, 2026, MyChart will be replacing Follow My Health. Most of your health information will transfer between systems automatically.
On Oct. 2, access to Follow My Health will be read-only. No new health information will be added and messaging will be disabled.
If you would like a personal copy of your information, you can download and save your data in the FollowMyHealth portal by using the export, print, or send features on a computer or mobile device:
- From the mobile app: Go to the "My Health" tab, click "Send" or the export icon, and choose to print, email, or export your files.
- From a computer: Go to the My Health tab, choose Summary, and click the Send option located at the top of the section. Select Print or Export to save the file as a PDF on your computer or choose Email to send a copy to your personal inbox.
We will share more details in the coming weeks, including when you can begin using MyChart and what to expect during the transition.

- Check personal medical information 24/7.*
- Review patient visit summaries and patient education materials related to services received at the SMH-Sarasota Campus, SMH-Venice Campus, our freestanding ERs, urgent care centers and other Sarasota Memorial facilities.
- View test results completed at SMH facilities.
- Set up a proxy portal account for children and dependent adults.
- View and pay bills; create payment plans; review past billing statements and payments; and set your preferred payment method.
- Access your account from any mobile device or computer with internet access.
- Go paperless.

If you've been a Sarasota Memorial patient, you can register for a FollowMyHealth Patient Portal account! There are two easy ways to get started:
- If you recently visited a Sarasota Memorial facility, check your email inbox for an invitation to register for a Patient Portal account. Simply follow the "click here" link in the email for step-by-step registration instructions.
- If you have not recently visited an SMH facility or did not receive an email invitation, you can still create a portal account and then request access to your records. Click here to begin the process.
